Join us as we celebrate the grand opening and reintroduction of Oak Creek Hospice under new ownership! We are proud to welcome Oak Creek Hospice as a valued partner of Choose Folsom. 

This special event will feature an Open House from 11:00 AM–4:00 PM, with delicious catering, refreshing drinks, swag bags, and plenty of opportunities to connect with the Oak Creek team and the community. Be sure to arrive in time for the official ribbon cutting at noon to help us commemorate this exciting new chapter. We look forward to celebrating with you!
